Upside-down manhole cover

A concerned citizen files a complaint about improper manhole-cover placement on K Street in South Boston:

After cleaning this catch basin they put the cover on upside down. Manhole cover needs to be flipped over. It's a tripping hazard the way it is.

Of course we need tic-tac-toe installations!

Just make them flat and safe to walk on and over.
Perhaps they could also be placed in heavily trafficked areas so that pedestrians can begin to reclaim the public thoroughfares in the center of the city!

I can see it now, "Boston 2024! First International Street Tic-Tac-Toe Competition"!

Why's that? A plain flat disc couldn't fall into the hole since the hole has a "ledge" around the edge.

I think the reason for the parts sticking up is to provide structural stiffness.
