Does the cargo get lifted into that gap? I mostly imagine them flying up and down, so it's a little hard to imagine that that's to reduce air resistance during forward flight or something. Is it just that they've omitted the usual enclosed seating and/or cargo space and that's the shape you're left with?

I’m not a Sky crane expert, but other than seating /interior cargo, what you are here are the important parts of a helicopter. Also, a cargo box / trailer would fit right in that spot

You can attach a big hopper for water or fire retardant under there, a shipping container (or mobile buildings, like field hospitals), even a bomb. Also, why add a bunch of structure and weight when the only thing the helicopter does is pick things up?
